How Diagnostic Imaging helped Ophthalmology team diagnose extraocular myositis

Oscar was referred to our ophthalmology service after his owners woke to find him with both eyes bulging prominently.

He underwent an MRI scan of his head, which allows detailed evaluation of the structures surrounding the eyes.

Using a dorsal plan STIR image, a type of MRI sequence which highlights areas of swelling and inflammation, we identified an intense signal arising from the extraocular muscles compared with other muscles of the skull.

This pattern is typical of extraocular myositis.

In this condition, there is presumed to be an immune-mediated attack on the extraocular muscles, with their distinct protein make-up ensuring that they become inflamed whilst other muscles in the body are typically unaffected.

Oscar responded well to treatment with a high dose of steroids, promptly regaining his good looks, and was gradually weaned off treatment over the following months.
